The best banner creator is photoshop Smiley and is not free Smiley . Anyway I found something in internet some script to ggenerate abannersbut was to weak to bookmark them Smiley

You can try requesting for a banner in a graphics forum. They usually do a good job if you specify exactly what you want and what you need.

I think is both. sometimes when i stuck with banner creative idea. I use photoshop gradient style to do experiment.  doing some trial and error. it is take some times but the result is not bad.
